Volunteer Forms: Please fill out the volunteer forms for background checks.
Hello Spring Hill Band Parents!!
Maury County requires any volunteer
to have a background check to be approved to travel with the students. We always
need as many as 12 chaperones for each away game and band contest.
Please help out by submitting the forms and volunteering to
chaperon.
It is a great way to meet other parents, support our kids, and GREAT time.
Information for Downloading, Completing & Submitting
the Forms
There are 3 documents needed for the background check:
1. A Waiver Agreement to check the background
2. A L1 information document
3. A Maury County Volunteer Application

When you have completed the forms, please return:
Forms 1 & 2 (Background Check) to: Angelina Reynolds areynolds@mauryk12.org
Form 3: Please place in a sealed envelope marked Volunteer
Application and return to Ms. Taylor or Mr. Stockdale
